How effective is propecia in halting hair loss if taken early in the process? Will it stop working after a certain number of years or does it usually stop all hair loss in the crown and mid scalp areas once the patient has started taking it? If I am on propecia to maintain my mid scalp and crown, but have already lost my temples and frontal regions of hair, is a transplant possible or is there to much of a risk that I will lose hair in the other areas if the propecia losses its potency? — M.

Propecia is very effective in halting hair loss if started early in the process. We have many patients who have been taking it for over a decade with no noticeable decline in effectiveness. We do not expect that one day the medication will all of a sudden stop working. If you are taking Propecia and it has effectively stopped or slowed the hair loss in the mid scalp and crown it should continue to do so indefinitely. To what degree your frontal zone and temples can be restored will also depend on your donor area and final hair loss pattern. In almost all cases if the patient is a surgical candidate, it is safe to restore the front and let the medication work on the mid scalp and crown.

Hi! I’m 23 years old. I have been facing hair loss for the last 3yrs and now my head at the corners and even at mid scalp is becoming noticeably thin. I have been taking treatment for the last 2 years (6 months from a small private skin specialist, 1 yr in Dr. —– hair clinic, now taking RichFeel). I’ve noticed hardly any improvement. Visit your local doctor or clinic and request a prescription for finasteride (I generally prescribe between 1mg – 1.5 mg per day). It may take up to 1 year to determine whether finasteride is effective for you. You’ll have to wait one year before any definitive assessment of the drug’s efficacy can be made. Some regrowth of hair may occur, but the main thing is that finasteride is very effective, in most cases, of at least stopping hair loss.

First, I’d like to thank you guys for putting together this blog. It’s very informative. My question was regarding Rogaine Foam. In your opinion, can it help keep hair on the hairline? I still have basically a full head of hair, but I’m starting to thin out on the hairline, and was wondering if using Rogaine would be worth a try. I’ve also heard about some people going through massive sheds when they first use Rogaine. Thanks so much! — J.M.

Thanks for your comments, J.M. Rogaine can cause a temporary shed. The hair will grow back in its natural growth cycle. This may take a few months or so. According to the Rogain website, shedding will likely occur in the first week of using Rogaine and by week 3, "the shedding should be slowing down". In our experience, Rogaine is not as effective as Propecia for stopping hair loss. Both drugs seem to be more effective in the crown than in the hairline where growth is concerned. For those who are not surgical candidates, we will sometimes recommend the use of Rogaine in the target areas of concern with the understanding that it may not produce results that meet with their expectations. The majority of patients we’ve seen who use Rogaine for the hairline see only the regrowth of peach fuzz hairs, but rarely enough regrowth to satisfy their expectations or produce a significant cosmetic difference. The downside of using Rogaine is the inconvenience of having to use it morning and night daily. Of course, any safe drug is worth trying, you may be lucky. Patient’s experience with the drug is variable, as you can see by reading this thread at Hair Transplant Network:
